Patient Recertification
What is Patient Recertification?
GO179 is physician service for the recertification of Medicare covered home health service.
How do Patient Recertification payments work?
Physicians who continue home health care every sixty (60) days receive payments from Medicare.
What are physicians paid?
The office par fee (reimbursement) for re-certification is $56.61 To receive payment, submit a signed copy of plan of care (485) along with HCFA Form 1500.
Services covered by Medicare are:
- Review of Medicare patient plan of care.
- Review of patient care data as it relates to home health care.
Rules for a successful claim per Medicare guidelines:
- A new certification (GO180) must be submitted before a re-certification can be claimed.
- HCFA Form 1500 must include the date, the GO179 charge of $56.61 and the home health agency provider number.
- A copy of the signed plan of care (485) must be submitted with the claim.
- The plan of care must be signed within thirty (30) days of the “From Date” located under the Certification Period heading found on form 485.
“Do Not’s” for filing your claim per Medicare guidelines:
- Do not submit a plan of care (485) that has been signed by another physician or physician assistant.
